How to Schedule a Cremation

Call us at Forever Pets
If your pet died at home, or is at another veterinary hospital, you can bring it to us to plan a cremation. Just call us at (606) 875-1359 to coordinate. Pets are dropped off at Midway Veterinary Hospital on Highway 27 in Somerset, Kentucky. A Forever Pets representative will meet you and help you with a small amount of paperwork.
At Midway Veterinary Hospital
If your pet is a patient at Midway Veterinary Hospital of Somerset, Kentucky, Forever Pets and the Midway staff will automatically take care of the arrangements. A Midway representative will help you with the paperwork.
Referral from a partner hospital
Forever Pets coordinates with several area veterinary hospitals. If your pet passes away at their facility and elect to utilize our services, your hospital will contact us to coordinate. All paperwork will be handled by your vet. Feel free to contact us with any questions.
